Thank you for choosing our services for your needs. Please read and sign the agreement below. It lays out policies related to billing, scheduling, cancellation procedures, and informed consents.

BILLING, SCHEDULING, CANCELLATION POLICY

  • Payment of all fees is expected at the time of service or via credit card on file.

  • I hereby authorize payment directly to Flor Verde for all services rendered where applicable.

  • Payments can be made via credit/debit card are due on the date of your appointment. Credit/debit card payments can be made directly with Flor Verde. I understand that I can request to make payments Paypal or Venmo by contact Flor Verde.

  • If I default on my account, I understand I will be subject to finance and/or legal fees in addition to the total account balance.

HOLISTIC NURSING, YOGA, PRANAYAMA, AND MEDITATION INFORMED CONSENT

The staff at Flor Verde are not healthcare providers or psychologists, and the scope of their consultation services does not include treatment or diagnosis of specific illnesses or disorders. If I, the client, suspect I may have an ailment or illness that may require medical attention, then it is my responsibility to consult with a licensed physician immediately. Only a licensed healthcare provider can prescribe drugs. Any change in prescription or dosage is a decision the client makes with their physician.

Any mention of drugs in the course of consultation is only for the purpose of providing a complete history of drugs that the client is taking and not for the nurses at Flor Verde to judge the appropriateness of the medication. As registered nurses at Flor Verde we primarily educate and advocate for clients to make informed health decisions. A nursing diagnosis differs from a medical diagnosis. A medical diagnosis refers to a disease process which is made by a physician and will be a condition that only a doctor can treat. In contrast, a nursing diagnosis describes a client’s physical, sociocultural, psychological, and spiritual response to an illness or potential health problem. For as long as a disease is present, the medical diagnosis never changes, but a nursing diagnosis evolves as the client’s response changes.

The goal as established in a nursing care plan – in terms of observable client responses – is what the nurse hopes to achieve by implementing nursing orders. It is a desired outcome or change in a client’s condition. The nursing care plan written by Flor Verde is not intended to diagnose or treat a disease. The care plan lists recommendations based on the client’s already established documented medical diagnosis and reported associated symptoms.

The nursing care plan is a shared decision model based on already established education and anecdotal information provided by others who have benefited from the use of medicinal cannabis.

By signing below, I acknowledge that I understand that the nurses at Flor Verde, are practicing as registered nurses and not healthcare providers and that I should see a healthcare provider if I think I have a medical condition. Flor Verde will not be held liable for failure to diagnose or treat an illness, nor will he/she be liable for failure to prevent future illness. Flor Verde is not responsible or liable for any advice, course or treatment, diagnosis or any other information, services or products that you obtain as a result of your consult. In addition, I have given Flor Verde a complete and accurate account of any medical conditions that I may have and any medications, supplements, and vitamins that I am currently taking.

TELEHEALTH CONSENT

I understand that my health and wellness provider Flor Verde wishes me to have a telehealth consultation or course.

This means that through an interactive video connection, I will be able to consult with the above-named provider about my health and wellness concerns. My provider has explained to me how telehealth will be used to do such classes or consultations and how else we will use telehealth to connect while working together, using the Healthie telehealth platform.

I understand there are potential risks with this technology:

  • The video connection may not work or it may stop working during the class or consultation.

  • The video picture or information transmitted may not be clear enough to be useful for the class or consultation.

The benefits of a telehealth class or consultation are:

  • I do not need to travel to the consult location.

  • I have access to a specialist through this consultation.

I have read this document and understand the risk and benefits of the telehealth class consultation and have had my questions regarding the procedure explained and I hereby consent to participate in telehealth sessions under the conditions described in this document.
